Acute aortic dissection following cannulation of the iliac artery.

چکیده
The aorta of a patient with aortic valve stenosis may be unduly thin, fragile, and liable to dissect irrespective of the presence or absence of atheroma. Indeed, Coleman (1955) stated that aortic dissection in young people was most commonly associated with coarctation or with bicuspid aortic valve. Petch (1952) reported fatal haemopericardium from ruptured dissecting aneurysm in a man of 35 years with calcific aortic stenosis; a similar example in a man of 65 years was described by Heath, Edwards, and Smith (1958). Hudson (1965) also refers to the association between aortic valve stenosis and aortic dissection, sometimes with Marfan's disease also. In the following fatal case this complication followed unsuccessful cannulation of the left external iliac preliminary to establishing cardiopulmonary bypass for surgical replacement of the diseased aortic valve.

Comparison of ascending aorta versus femoral artery cannulation for acute aortic dissection type A.
BACKGROUND The site of cannulation for repair of ascending aortic dissection remains controversial. We present our experience with ascending aortic cannulation for acute aortic dissection type A. METHODS AND RESULTS From January 1988 to September 2007, we operated on 242 patients for acute aortic dissection type A. Medical records of 235 patients who received ascending aortic cannulation or f...
متن کاملAortic dissection presenting as acute lower extremity ischemia: report of a case.
Although not common, acute leg ischemia is an important element in the clinical presentation of a patient with aortic dissection. This report describes a case of aortic dissection in which the main feature at presentation was acute right leg ischemia. The angiography showed right common iliac artery and external iliac artery occlusion. Diagnosis was made by clinical evaluation and angiography. ...

We report a successful treatment of unusual case of a 48 year old male patient with acute aortic dissection type Stanford A that expanded into left common and external iliac artery diagnosed by MSCT angiography, presenting as a single leg paresis, without symptoms of a chest or back pain. Patient was operated with conventional ascending aortic replacement. Patient had no known prior medical con...
متن کاملComparison of the left ventricular apex versus other arterial cannulation sites for the operative management of acute type A aortic dissection
Method Between January 2010 and January 2015, emergent surgical aortic repair was performed in 158 consecutive patient with acute type A aortic dissection. In all patients open distal anastomosis was performed using deep hypothermic circulatory arrest. Patients were divided into two groups: transapical cannulation group and other cannulation sites group (including femoral and axillary artery ca...
متن کاملHow I do it: transapical cannulation for acute type-A aortic dissection
Aortic dissection is the most frequently diagnosed lethal disease of the aorta. Half of all patients with acute type-A aortic dissection die within 48 hours of presentation. There is still debate as to the optimal site of arterial cannulation for establishing cardiopulmonary bypass in patients with type-A aortic dissection. Femoral artery cannulation with retrograde perfusion is the most common...
